Lagu ini dirilis pada tahun 1970 dan dimuat dalam album dengan judul yang sama, yaitu Paranoid. Lagu ini ditulis oleh keempat anggota dari Black Sabbath. Sang bassist, Geezer Butler, mengatakan bahwa lagu ini menggambarkan depresi. Hanya saja ia tidak bisa membedakan antara depresi dengan paranoia sehingga ia lebih memilih menggunakan tema paranoia--meskipun dalam liriknya tidak tertera kata 'paranoid'. Ia mengatakan bahwa masalahnya bersumber dari narkoba yang membuat seseorang menjadi berpikiran tidak waras dan depresi.
